TASO Officials that experience incidents at UIL and TAPPS competitions must notify appropriate TASO Chapter Officers and file the necessary reports with UIL or TAPPS as soon as possible and no later than 24 hours after the incident. SAMPLE INCIDENT REPORTS
PLAYER EJECTION - SECOND CAUTION
John Doe (Lopez #4) was disqualified in the 68th minute for receiving a second caution in the same
match. Mr. Doe was first cautioned in the 15th minute for encroachment. Mr. Doe was then cautioned in
the 68th minute for unsporting behavior.

the forward and the goal resulting in Mr. Doe denying an obvious goal scoring opportunity.

COACH EJECTION
Coach John Doe for Pace High School was disqualified at the 57th minute for using insulting language
towards the game officials. Coach Doe asked repeatedly, "How much is the other team paying you
cheating referees to call all the fouls against my team?" Coach Doe was shown a red card and asked to
leave the vicinity.

Jack Nicholson was asked to leave the vicinity of play at the 55th minute for standing behind the goal
and yelling obscene comments at the Pace Goalkeeper. Mr. Nicholson was calling the Goalkeeper
"Hunchback" whenever she got near the ball. The Hanna Administrator escorted Mr. Nicholson from the
stadium.

The match balls presented did not have the required "NFHS Authenticating Mark". Both teams were
asked for replacement match balls, but no balls had the required markings. A set of balls were selected
that were safe and the game was played.
